It is suggested that you also keep a photocopy of these for future reference. A short quiz will be given during class time as we complete each chapter |

For students under the age of 18, parental signatures are also required. When filling in the medical history questionnaire printed on the inside of your student file folder, be sure to clearly write “YES” or “NO” next to each item. Do not use check marks or X’s, do not draw lines down the columns. |

Equipment Preparation and Checklist |
|
Scuba Mask*
(available for rental $15) |
Professionally fitted, scuba-grade mask. Prior to use, your mask lenses should be cleaned as follows: Apply toothpaste to a soft cloth and scrub the inside and outside of your lenses, then rinse. This one-time cleaning will allow your mask to be more easily de-fogged. Dive mask lenses must be tempered safety glass and are labeled with the word “tempered” or the letter “T” as an indicator. DO NOT scrub lenses made from other materials as they will scratch. |
|
Snorkel *
(available for rental $5) |
Self-draining, scuba-grade snorkel. Attach your snorkel to the left side of your mask strap and adjust it to fit comfortably |
|
Scuba Fins*
(available for rental $20) |
Scuba-grade, open-heel, adjustable fins |
|
Neoprene dive boots*
(available for rental $10) |
Neoprene dive boots should fit comfortably inside of your fins without compressing your foot. Improperly fitted boots and fins can cause cramping |
|
Swim suit & Towel |
|
|
Lunch |
There will be a lunch break between classroom and pool sessions. You may choose to bring a ‘brown bag” lunch or otherwise purchase lunch at the Y’s on-premise café. Be sure to bring along sufficient food and drink to be comfortable through the course of the day. |
Major Equipment: Included in your course tuition is the use of the following major pieces of diving equipment: SCUBA air cylinder, buoyancy compensating jacket, regulator system, weight belt, and weights.
Basic Equipment can be fitted and rented* directly from your instructor for the course duration. Please note, there may be some limitations on rental equipment availability for individuals with unusual sizing requirements. If you choose to provide your own basic equipment, be aware that improperly fitted equipment can adversely impact a diver’s comfort and safety. Equipment should be “scuba” quality and professionally fitted by a local diving retailer. “Sport-grade” equipment, such as you might purchase from a general sporting goods store or department store is not suited for scuba diving. Internet and catalog purchases are also not advised as there is not opportunity to ensure correct fit to the bone structure of the diver’s face and feet.
